Transitions ACTIVE/SUSPENDED → REVOKED. Idempotent on already-REVOKED → 204 (NOT 404, NOT 422) — matches Actor Roles unassign convention. Soft-deletes the aggregate; audit history preserved. Scope: admin.tenant.internal-use-case.revoke
